Which One Do I Fill Out?

Hazard Report

Something could cause harm, but hasn’t yet — e.g. a damaged target stand, a trip hazard, worn equipment.

Near Miss Report

Something happened that could have caused harm, but didn’t — e.g. an arrow landed close to someone, a piece of equipment nearly failed.

Incident Report

Something happened and someone was hurt, or property was damaged.

Observation Report

A general safety observation worth noting — not urgent, not necessarily a hazard, just something worth recording for the club’s awareness.

Not sure which one applies? Fill in whichever feels closest — any report is better than none, and it can always be reclassified by whoever reviews it.

What to Have Ready

• Your name and role (member, visitor/guest, or other).
• Date, time, and location (which range, Club Day, session, or event).
• What happened or what you observed, in your own words.
• Who else was involved or nearby.
• Any initial action already taken.
• Photos, if you have any and it’s safe to take them.

How to Submit

• Complete the form below.
• Completion will submit it to the club committee for follow on action.

Reporting isn’t about blame — it’s how the club catches small problems before they become big ones. If something felt off, it’s worth writing down.
